Students working in labs or on campus as part of an MAE program, but without official Cornell status (i.e., not registered for a course or employed by the University), should register as a Cornell University Volunteer.  [Non-Cornell students working here without official status may have different requirements.  You should contact Andy Vail for these situations.]

The steps needed to register as a University Volunteer are:

 

1. On the Risk Management web page are a few University forms that MUST be completed: http://www.treasury.cornell.edu/risk/library/forms.asp

 

a. Description of Volunteer Services Form

b. Volunteer Profile

c. Volunteer Agreements

 

2. The following M&AE forms MUST be completed as well:

 

a. MAE Volunteer Form

b. Emergency Contact Cards

1a) Description of Volunteer Services Form


The person administering or working with the volunteer needs to fill out a Description of Volunteer Services.  This form defines the volunteer's role while working on campus.  Students (especially project team students) can fill these out themselves and have their advisor sign them.  There is no space for the advisor to sign the form, so the advisor should just sign the bottom indicating they have approved the information. 

 

1b & c)Volunteer Profile and Volunteer Agreements


The volunteer needs to complete the Volunteer Profile and Volunteer Agreements.   The Volunteer Profile provides info on the student.  The Volunteer Agreement defines Cornell’s and the student’s responsibilities regarding insurance, pay, etc.  It is important that the student clearly understand that they will have no workman’s compensation or health insurance coverage while a volunteer.  We recommend that you discuss this with your volunteers.

2a) MAE Volunteer Form

 

Volunteers working in MAE need to fill out the MAE Volunteer Form.  This form collects information on expected work dates and hours per week.  It also asks for health insurance information.  [Students without valid health insurance will not be allowed to work on campus.]  In addition, it asks the student to certify that they are in good standing at Cornell and not on any kind of leave from the University.

 

 

2b) Emergency Contact Cards

 

These are cards that we will hold in Arlene’s office for quick reference in case of emergency.  They should be filled out by the students.

 

3)  All the volunteer forms should be given to Arlene Meddaugh in 108 Upson.  She will file copies for our records and send Risk Management their required forms.
